A: The review only indicates that other studies (supposedly, “well designed”) have found that ciclesonide is superior to placebo on two measures–sx improvement x duration and quality of life. The latter has been deemed not clinically relevant.

Quite erroneous to extend these findings and contend that ciclesonide is superior to all other ICS’s. I think this is, at the very least, indirectly implied, here.

Truth is, most ICS’s, including fluticasone propionate and budesonide, are inhaled in their pharmacologically active form. Ciclesonide (as well as beclomethazine diproionate) are different in that they are inhaled as inactive compounds and are converted to their active forms within the lung. Sooo, only implication here is a potential reduction in local and systemic side effects. But, is ciclesonide (and beclomethazine dipropionate) more efficacious than other ICS’s? No.

There is also evidence that the hypotonic ciclesonide suspension provides higher intracellular concentrations of des-CIC (up to 24 h) when compared to isotonic suspension. Again, this does not imply it is more efficacious. It’s just more “convenient” via the requirement of less frequent dosing:
